Once you submit the form we will contact you by email containing further instructions and a Return Authorization Number. Once you receive a Return Authorization number, ship the item(s) to be returned to:

We must receive the shipment within 10 business days of the issuance of the Return Authorization number. You can choose to ship the items yourself, or have us email you a Priority Mail return label for a charge of $3.99. If you choose to ship the items yourself you are responsible for the shipping cost of returning the item. Regardless of the shipping method chosen, you are responsible for any damage to the item that occurs in transit, so please handle and package the item carefully and purchase insurance at your discretion.
